The position of Client Services and Testing Counselor provides individual counseling, HIV/Hepatitis screening, testing, and care coordination to the IDU and harm reduction population.

Client Services and Testing Counselor

Responsibilities

Conduct HIV/Hepatitis pre and post test counseling.
Conduct HIV and Hepatitis C testing for program participants
Provide HIV and Hepatitis harm reduction counseling to program participants.
Take a leading role in the Hepatitis C and HIV counseling and care coordination under the IDUHA and SEP.
Conduct Hepatitis and HIV treatment readiness counseling sessions.
Connect HIV/Hepatitis positive clients to treatment and care including referrals and follow up.
Coordinate holistic services for clients as needed.
Provide support for peers on testing and program guidelines.
Conduct full intake assessment of needs to new program participants.
Complete Service Plans based on presented needs.
Support program staff to SEP daily transactions and services.
Escort clients to linkage to care as needed
Participate in periodic case conferences and chart reviews.
Navigation and Data entry tasks through agency databases. Including and not limited to AIRS, Eshare, EiCare, and ECW.

Qualifications

Minimum of an Associates Degree and/or three years of case management experience.
Bi-lingual (English/Spanish) a plus
Experience and training in Syringe Exchange programming
Harm Reduction approach
Knowledgeable about HIV and Hepatitis services
Pre and Post Test counseling
LGBT sensitivity
