Services & Advocacy for GLBT Elders (SAGE) – New York, NY
The primary function of the E-LINC (Elder LGBT Interprofessional Collaborative Care Program) Social Worker is to serve as point of contact for SAGE program participants and clients, and the E-LINC Program which is operated and coordinated by the Columbia University School of Nursing. The role of the Social Worker in this intercollaborative model of service delivery is to provide: Mental Health Assessment, Counseling, Dementia Care, Family & Caregiver Support and Case Management
The E-LINC Social Worker will follow up with all internal referrals to the E-LINC program and perform assessment and intake upon clients to help them in accessing E-LINC health & wellness programs at SAGE. The E-LINC Social Worker will provide counseling and make mental health referrals to participants in the program, primarily to the Lucy Wicks Clinic and other providers. This position will meet with and liaison with the Nurse Practitioner and other staff in the E-LINC program to assist with the production of wellness programs at SAGE, and will arrange for those services to be incorporated in other Care Management services through referrals and home visits. The E-LINC Social Worker will additionally perform case assistance and information & referral in the SAGE Centers Midtown and Harlem, as well as supporting SAGEDay and SAGECap, and will carry a case load of clients and will do all required recording and reporting for these various functions. The E-LINC Social Worker reports to the Director of Care Management Services & receives group clinical supervision from the Senior Director of Programs. The E-LINC Social Worker must be a LMSW and have a working knowledge of aging and /or LGBT communities. She/he must be able to work independently as well as be a major team player, and interact with, relate to and work with LGBT older adults, community partners and other departments within SAGE.
ELINC Social Worker Care Management
Essential Functions:
- Perform intake and assessments for people referred to the E-LINC program
- Provide case management, information and referrals to participants in the E-LINC program
- Make home visits and help assess homebound SAGE clients for E-LINC services
- Provide counseling and make referrals to Lucy Wicks Clinic and other mental health providers to participants in E-LINC program
- Meet with Nurse Practitioner and other E-LINC staff to plan E-LINC programs and assist with and support program production in different SAGE locations
- Possibly design, launch and run support groups or workshop seminars as defined by identified E-LINC program gaps or needs
- Be primary link to SAGEDay Program and act as back-up for SAGE Center Midtown case manager
- Keep a caseload of clients, assist with benefits application processes and perform advocacy as needed
- Collect all participant and service unit data as required by programs
- Enter all participant and service unit data in database as required by programs
- Complete all administrative paperwork and assist in reporting process as required
- May be assigned other tasks and duties reasonably related to job responsibilities
Position Requirements:
- Education: LMSW
- Experience: Two (2) – Three(3) year’s experience preferred with aging and/or LGBT communities
- General interest in and knowledge of LGBT aging health issues
- Bilingual (English/Spanish) preferred
- Able to utilize client data bases
Related Skills or knowledge:
- Familiarity with social and aging services networks and supports
- Strong organizational skills
- Computer-literate
- Familiarity with LGBT aging issues preferred
- Ability to multi-task in fast-moving environment
- Ability to work individually and as a member of a team
Other Requirements:
- Flexible and dependable – This position requires some flexibility around scheduling. Some very occasional evening or weekend hours may be required with advance notice.
- Travel – This position requires some travel via public transportation to different SAGE Center sites in New York City, primarily Harlem, but also Brooklyn, Bronx and Staten Island.
