For over 30 years the Alliance for Positive Health has modeled client centered care and provided skills to help those we serve make informed decisions. At the Alliance for Positive Health, we work with those who face barriers to accessing health care and social services and strive to reduce health disparities. Our clients’ diagnoses are often complicated by stigma, addiction, poverty, homelessness, incarceration or mental illness – and these are the factors which also place them at risk of infection with HIV, Hepatitis C and other Sexually Transmitted Diseases.
We provide care coordination for people with chronic diseases including HIV/AIDS, diabetes, hypertension, heart disease, obesity, serious mental illness and asthma. We work in partnership with our clients to help them navigate complex systems. We provide direct assistance and skills building to help them become more independent and successful in their disease management.
We prevent new cases of HIV, Hepatitis C and Sexually Transmitted Infections (STIs) by reaching those who have the highest chance of exposure, raising awareness of personal risk, conducting thousands of HIV, STIs and Hepatitis C tests each year, and reducing the stigma and discrimination which continue to surround these diseases and the people who are most impacted by them. We believe information and advocacy help clients better access health related services and enables them to make informed, healthier decisions.
Our core services are offered in the 15 counties of Northeastern New York, stretching south to Columbia and Greene Counties, west to Schoharie, Fulton and Montgomery Counties and north to Clinton and Franklin Counties. We also offer enrollment in the New York State Health Exchange in Ulster, Dutchess and Delaware Counties and HIV transportation in Delaware and Otsego Counties.
As a legacy provider of the AIDS Council of Northeastern New York, we have a special commitment to ending the AIDS epidemic and being a resource for all who are living with HIV/AIDS in our region. There are thousands of people in New York State who do not know they are infected; more people are living with HIV/AIDS than ever before. We must work to make sure all those who are infected know their status, link them to early care and treatment, and prevent new cases from occurring.
