Tri-City Health CenterFremont, CA

To coordinate and integrate primary medical care, patient education, and treatment adherence for HIV+ and all patients, to interface with other entities including specialty care providers, pharmacies and other Bay Area clinics and hospitals to ensure access and continuity of care. Provide integrated health and HIV Specialty services within same clinic location including patient assessment and routine lab procedures. Coordinate specialty care and other needed services for patients and ensure follow-up with health Care Plan. Provide patient education individually and in group settings

Participate in bi-weekly case conferences with HIV Care Team and work in collaboration with Case Managers and Client Services Advocates to maintain HIV clients in care, establish maximum use of client entitlement and insurance programs, and coordinate the flow of services for each patient. Represent agency at HIV ACCESS network meetings and at other meetings required by funders of HIV medical program. Maintain documentation for case reporting as required.

Essential Responsibilities
  • Provide medical services and integrated specialty HIV care, including patient assessment and routine lab procedures.
  • Provide preventative health services, counseling and education utilizing TCHC internal referral system as necessary.
  • Provide appropriate information to HIV patients regarding diagnosis, treatment plan and follow-up, and participate in tracking of patients for return appointments.
  • Provide patient education regarding treatment and other related issues for living with HIV in one-on-one and group sessions.
  • Coordinate specialty care and other needed services for patients and assure follow-up with medical Care Plan.
  • Participate in bi-weekly case conferences with HIV Care Team and work in collaboration with Case Managers and Client Services Advocates to maintain HIV clients in care, establish maximum use of client entitlement and insurance programs, and coordinate the flow of services for each patient.
  • Represent agency at HIV ACCESS network meetings and at other meetings required by funders of HIV medical program.
  • Establish and maintain relationships with pharmacies, hospital emergency rooms and other Bay Area providers in order to assure access to services.
  • Work with HIV Program Manager to assure appropriate documentation and data collection for HIV Clinic activities, adequate access to appointments for patients, and other services related to the comprehensive care of patients within the HIV Services Program.
  • Participate in the HIV ACCESS and Alameda County Office of AIDS Continuous Quality Assurance programs, as well as QA activities within Tri-City Health Center.
  • Maintain documentation for HIV Case Reporting as required by Alameda County, State Office of AIDS, and Centers for Disease Control guidelines and regulations.
  • Work flexible schedule and overtime, as necessary.
Secondary Responsibilities
  • Attend workshops, training, and meetings as needed, and as requested.
  • Perform other duties as assigned by supervisor.
Qualifications
 
Required education, experience, training
  • Registered Nurse with current and valid California license.
  • Minimum of one year prior work related experience in medical clinic environment
Preferred education, experience, training
  • Experience working with patient management software programs
  • Community health center work related experience
  • Experience in Quality Management
  • Bilingual; Spanish
  • Prior work related experience with HIV+ populations, active substance users, homeless individuals, and other vulnerable group
Certifications/Licenses:
  • Current Basic Life Support Certificate
  • Access to reliable transportation

  • This position requires the ability to work closely with target populations including, but not limited to HIV+,       active substance users, homeless, LGBT community.
  • The ability to gather data, compile information, and prepare reports.
  • Presenting information in front of a group.
Physical Demands:
This position requires:
  • Sitting, walking, and standing associated with a normal medical/clinical office environment; regular bending and stretching; and lifting.
  • Manual dexterity used to include, but not limited to, standard office equipment, and computer keyboard.
  • Reading and writing in order to review records for accuracy.
  • Ability to receive detailed information through oral communication and express or exchange ideas  through spoken word.
  • Moderate physical activity. Requires handling of average-weight objects up to 20 pounds or standing and/or walking for more than 4 hours per day.
  • Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment.
  • Will work with blood or blood-borne pathogens and will require OSHA training.
