HIV Case Manager: Will be involved protocol development, resource identification, collection of resource utilization and patient outcomes data, as well as provision of client centered, wrap around case management to individuals living with or at risk for HIV. As part of the process they will assess and triage the social and psychological needs of the patient and their family and develop comprehensive service plans; conduct initial assessments/reassessment of client needs, implement service plans, identify gaps in services and provide ongoing follow up; advocate for clients and identify obstacles to compliance/adherence; maintain documentation of services and keep statistical reports; attend interdisciplinary team meetings, mandatory trainings, participate in pager coverage, and community outreach/education; and make home visits (car required) as needed. Case managers will be expected to maintain a case load of 25-40 individuals and will actively facilitate connections with other CBOs.

The candidate will provide wrap- around case management services to individuals who are HIV positive or are at risk of acquiring HIV. The case manager will maintain a caseload of 25-40 individuals. Provision of services will require: linkage to HIV and PrEP care; referrals to other community based organizations (CBOs) and social service agencies (e.g. housing, food, and primary care organizations); follow up with participants of the program; monitoring and evaluation of services; documentation of client encounters and services. This individual will also: collect and analyze data to identify program progress and any need for change; work closely with the PI and other project coordinators; collaborate with community members and Chicago Department of Public Health (CDPH); assist in the development of protocols and procedures for the network based HIV/STI testing program and linkage to care; actively pursue required knowledge and skills for professional development; stay informed about the CDPH regulations and requirements; case manager may conduct data analysis and provide appropriate recommendations or conclusions for the program.

Principal contribution is the application of principles and knowledge relevant to the administration and support of the research project. Under limited direction, performs complex lab and/or research-related duties and tasks.

  • Helps develop, design and conduct research projects according to plan, including planning new procedures, adapting existing procedures to the needs of the project, and making contributions to methodology.
  • Collects, records and analyzes data.
  • Conducts literature reviews. Participates in writing reports and manuscripts. Oversees the management of equipment and supplies.
  • Ensures compliance with institutional, state, and federal regulatory policies, procedures, directives, and mandates. Trains and oversees the work of research support personnel.
  • Responsibilities may include the following non-laboratory duties: transcribing and coding data; developing data collection instruments; presenting research findings at staff meetings, seminars, and scientific conferences; assisting with building databases; and recruiting and scheduling research subjects.
  • Responsibilities may include the following laboratory duties: recognizing abnormal results and varying conditions or procedures to correct problems; setting up and performing experiments. May serve as a lead research specialist in smaller labs.

Education

  • Bachelor’s degree required.
  • Bachelor’s degree in public health science (epidemiology, community science), social work, or related field preferred.

Experience

  • Required: Experience working with and/or providing care to LGBT populations, Black and Latino community members; previous work experience providing HIV/STI treatment and/or prevention; previous case management experience.
  • Preferred: Previous work experience with Chicago Department of Public Health; previous supervisory experience; previous grant writing experience; program management and/or development experience; program monitoring and evaluation

  • Ability to travel to various community venues across the metropolitan area
  • Ability to manage a case load of up to 50 patients
