Office Grant Manager

The Hebrew Home at Riverdale (HHAR) and Services & Advocacy for GLBT Elders (SAGE) were recently awarded funding by the New York State Department of Health to establish the nation’s first Medicaid eligible, LGBT centered social model adult day health program. Set to launch in October 2014, the program model will focus on education, promoting awareness and providing direct care and support to Medicaid eligible LGBT older adults in New York City.

To that end, HHAR and SAGE are looking to bring on a team of dedicated and dynamic employees to launch this historic initiative. The Office/Grant Manager coordinates day to day activities related to the clerical aspects of the program such as transportation, monthly census reports and all data/reports pertaining to grants.

Essential Functions:

  • Projects weekly schedule for participants in SDC program.
  • Maintains and distributes monthly census reports pertaining to Grant.
  • Creates and updates personnel and participant roster
  • Types memos for Check request, purchase requisitions, supplies request and maintain petty cash log for program.
  • Communicates with transportation dispatcher on participant schedule changes.
  • Maintains admission and discharge logs – communicates changes to appropriate personnel
  • Responsible for preparation and distribution of all written material generated by Site Manager
  • Maintains files for all program expenditures.

Qualifications:

  • At least 1 year experience working in Adult Day Health (social or medical model) or related environment preferred
  • Experience working with older adults and/or in the LGBT community preferred

Education:
Associates degree (A.A.S.) or equivalent from two-year college or technical school.

Related Skills or knowledge:

  • Excellent interpersonal skills
  • Excellent written and verbal communication skills.y NOW
  • Self-starter with good organizational and time management skills.
