The LGBT Health Resource Center (LHRC) is partnering with Services and Advocacy for GLBT Elders (SAGE) to replicate SAGE’s successful Caring And Preparing (CAP) program, which links LGBT caregivers with supportive resources. Under direction of the SAGECAP Program Manager, the Outreach Counselor will be the liaison to the community at large for promoting, enrolling, and counseling participants in the CAP Program. The position will have direct responsibility for executing aspects of the marketing plan, raising awareness of LGBT older adults and caregivers, and bringing new and existing clients into the support systems of the LHRC and Chase Brexton Health CARE. The Outreach Counselor will provide individual and group support services to care partners participating in the program.
Outreach Counselor SAGE CAP
MAJOR DUTIES AND RESPONSIBILITIES:
- Implementing SAGECAP programming and trainings with participants and volunteers, including but not limited to: providing one on one support sessions for care givers, hosting and facilitating support groups; coordinating educational sessions; linking participants to resources.
- Link SAGECAP participants with the intake processes of CBHC and other services of the LHRC. Work with CBHC staff and SAGE in the design and execution of events, educational opportunities, and seminars to reach the greater Maryland LGBT community
- Market and represent the LHRC to the surrounding community, building strong and effective referral networks
- Assessment and recording of all participants for current needs
- Have a working knowledge of the services opportunities provided by the LHRC and CBHC to refer participants into the available services
- Plan and coordinate both internal and external events as they relate to program needs
- Other duties as assigned
SKILLS AND ABILITIES:
- Ability to work effectively and respectfully as part of a multidisciplinary culturally diverse staff
- Ability to build and maintain effective relationships with clients, community partners, and employees of Chase Brexton Health Care
- Ability to facilitate a group discussion
- Knowledge of LGBT Issues
- Knowledge of medical issues, particularly around formal and informal care giving, and geriatrics
- Ability to work with diverse populations
- Ability to represent LGBT Health Resource Center of Chase Brexton Health Care in the community
- Ability to lift 30-50 lbs
- Availability to work evenings and weekends for program related events
EDUCATION AND/OR EXPERIENCE:
- Bachelor’s degree in Social Work, Aging Services, Health Administration, or related field required
- Community, case management, or outreach experience
- Marketing experience preferred
- At least 1 year of prior job related experience in a health care setting preferred
