The Patient Navigator will serve in a critical role as a member of the Elder LGBT Interprofessional Collaborative Care Program (ELINC).

Under the Direction of the ELINC Project Director, responsibilities will include, but are not limited to:

-Serve as a point of contact for patients in the community.
-Accompany patients to medical appointments.
-Enroll program participants.
-Assist ELINC health team members with providing patient education and support.
-Assist with data collection and reporting
-Assists the ELINC team in identifying patient needs and helping with referrals.
-Coordinates scheduling of ELINC field-based team.
-Establishes and maintains positive relationship with participants, their providers, and the ELINC team.
-Participates in interprofessional collaborative team projects and meetings.
-Able to serve as a liaison with other community-based LBGT and older adult programs.
Other related duties as assigned required.

This is a 3 year grant funded position.

Requires a bachelor’s degree or equivalent in education, training and experience, plus two years of related experience.

Applicant MUST meet these minimum qualifications to be considered an applicant Experience in a health or community-service field.

Excellent written and verbal communication skills.
Excellent organizational skills and computer skills.
Familiarity with LGBT health issues and or older adult service.
Preferred Qualifications Bi-lingual, Spanish-English
