The Treatment Advocacy Coordinator provides prevention case management to HIV positive gay, bisexual, and transgender (GBT) men; works with the Positive Force Program Manager to recruit and retain clients; and coordinates events under the supervision of the Positive Force Program Manager. Positive Force is a peer-based program by and for HIV positive GBT men. Positive Force helps a diverse group of HIV positive GBT men live healthy and meaningful lives by offering individualized case management, group treatment adherence counseling, and social and educational opportunities.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Include the following. Other duties may be assigned.

  •  Case Management: Provide one-on-one prevention case management ( PCM) to HIV positive GBT men who are experiencing issues with treatment adherence, engagement in care, and integrating HIV into their lives, in an effort to help them achieve virologic suppression and overall better health outcomes. PCM may be provided in-person, via phone, or virtually, depending on the needs of the client. Maintain accurate and timely case notes.
  • Recruitment: Identify, recruit, and retain HIV+ GBT men who struggle with adherence into the program by working with private physicians, other ASO’s, community clinics, DPH partners, bars, sex clubs, gyms, and online dating and “hookup” sites and apps, among other identified venues where the target population can be reasonably reached.
  • Coordination of Events: Coordinate monthly social events for program participants. Support peer volunteers.
  • Program Assistance: Assist the PF Program Manager with quarterly educational treatment adherence forums. Work with PF Program Manager to develop measurable process and outcome objectives for secondary prevention activities and interventions. Support the PF Program Manager in conducting PLUS Seminars for newly diagnosed/newly dealing HIV+ men, conduct PLUS Seminar intakes and follow-ups.
  • Group Facilitation: Lead groups on health and other personal growth topics related to, but not limited to HIV/AIDS and other issues related to wellness and adherence.
  • Referrals and Resources: Develop strong referral relationships with staff at partner ASO’s.

Other Skills and Abilities: Excellent oral and written communication skills required. Outgoing personality with proven ability to speak comfortably in front of large groups. Comfort with sexually explicit material and language. Able to act on initiative, identify opportunities and be proactive in putting forward ideas & solutions. Ability to prioritize and work under short deadlines. Willingness to work in a team-based, multi-collaborative environment. Deep knowledge and comfort with HIV positive issues and the SF LGBT community and concerns. Knowledge of San Francisco’s HIV continuum of care system for HIV-positive individuals. Understanding of social norms theory and promotional marketing. Proficiency in Microsoft Word, Excel and Access Database. HIV test counselor experience and/or other peer-based support training preferred.

EDUCATION AND/OR EXPERIENCE:

Bachelor’s degree preferred, and 2 years of counseling and/or public health experience or an equivalent combination of education and experience required. Fluency in English and Spanish preferred.

PHYSICAL DEMANDS:

Skill in operating office equipment such as a personal computer, copy machine and telephone system. Ability to perform routine bending, stooping, twisting, and reaching. WORK ENVIRONMENT:

SFAF: This position is primarily located at 1035 Market Street, San Francisco, however may require traveling to other office locations or off-site meetings as required by the job.
