The Population Health Specialist is responsible for improving WWH’s population health outcomes by optimizing the use of the Electronic Medical Record (eCiinicaiWorks) and the use of patient data registries. This is an ideal position for a public health or health informatics generalist who is interested in the multi-faceted and continuously transforming environment of community-based primary care.
For nearly four decades, Whitman-Walker Health has provided health care services for the LGBT community. In the 1980s, we became a leader in the fight against HIV/AIDS. And, in the 21st Century, we have evolved into a full-service community health center.
Population Health Specialist
- Serves as the health center resource for EMR (eCiinicaiWorks) configuration and best practices
- Serves as the health center resource for Ryan White Careware software configuration and best practices
- Serves as the health center resource on EMR-based communication and engagement tools such as patient portal, questionnaire, kiosk, patient recall, mobile app, and messaging tools.
- Maintains disease registries and reviews panels at regular intervals to facilitate proactive interventions with clients who meet determined clinical criteria (preventive and chronic disease)
- Develops data reports, training modules, and written procedures to support clinical and operational quality improvement initiatives across the health center
- Supports health center population health initiatives, such as CMS’s Meaningful Use incentive program and NCQA’s Patient-Centered Medical Home program
- Supports health center clinician peer review activities by designing review tools, compiling results, and conducting chart audits
- Participates in projects to improve the quality of patient data and data collection methods
- Assists the program/clinical staff in the development and implementation of program protocols and guidelines according to evidence-based standards of care.
- Undertakes quality improvement projects as directed
Knowledge, Skills and Talents Required
- Knowledge of EMR configuration and EMR registries. Specific knowledge of eCiinicaiWorks EMR required. Knowledge of Ryan White Careware software a plus.
- Developed understanding of how data informs quality improvement processes.
- Well-developed statistical and analytical skills, including the ability to prepare accurate reports using data extraction and analysis tools, such as SAS, SQL, MS Excel, MS Access, and EMR registries.
- Knowledge of ambulatory community-based primary health care, HIV prevention and care, and LGBT health care.
- Familiarity and experience with the principles of continuous quality improvement, such as PDSA cycles, root cause analysis, and graphic representation of data.
- Highly developed presentation and interpersonal skills, including the ability to train clinicians and patients alike on EMR best practices and quality improvement initiatives.Specific experience with CMS Meaningful Use reporting requirements and NCQA Patient-Centered Medical Home reporting requirements highly desirable.
- Intermediate level Excel, PowerPoint, Access, Word, and Outlook skills.
- Sensitivity to all areas of diversity, including HIV status, race, ethnicity, ability, age, sexual orientation and gender identity.
Education and Experience Required
- Bachelor’s degree required. Master’s degree in Public Health, Health Services Administration, Health Informatics, or the equivalent (in hand or in process) required.
- Prior experience with data management and quality reporting required, preferably in a Community
Health Center and/or Federally Qualified Health Center (FQHC) setting.
