The LGBT Health Resource Center (LHRC) is partnering with Services and Advocacy for GLBT Elders (SAGE) to replicate SAGE’s successful Caring And Preparing (CAP) program, which links LGBT caregivers with supportive resources. Under the direction of the LHRC DPD, the Program Manager will be responsible for the successful implementation of the CAP program in the greater Baltimore Area. The position will be responsible for assessing, promoting, enrolling and counseling participants in the SAGECAP Program. The CAP Program Manager will need to maintain good working relationships with the staff of CBHS to identify opportunities for SAGECAP Participants to engage with available services.
Program Manager SAGE
MAJOR DUTIES AND RESPONSIBILITIES:
Work with the SAGE Replication Coordinator to create, adapt and coordinate counseling and engagement events for care partners, including but not limited to care needs assessment, support groups, one on one counseling, and life planning
Implementing the created CAP programing
Create a welcoming space for LGBT elders and care partners
Link participants to Chase Brexton Health Center resources for care and support
Develop and maintain current literature and materials for participants
Work with the Director of Program Development to increase awareness of LGBT older adults
Track and trend participant data and outcomes
Conduct evaluations of programming outcomes
Represent the LHRC in the community
Build relationships with the LGBTQ, Aging, and Caregiving communities
Provide supervision and guidance for the Community Outreach Counselor
Respond to the needs and opportunities presented by participants
Plan and Coordinate both internal and external events related to Program needs
Other duties as assigned
SKILLS AND ABILITIES:
Ability to work effectively and respectfully as part of a multidisciplinary, culturally diverse staff
Ability to build and maintain effective relationships
Ability to facilitate a group discussion
Knowledge of LGBT Issues
Knowledge of Medical issues, particularly around formal and informal care giving, and Geriatrics
Ability to work with diverse populations
Ability to represent CB LHRC in the community
Experience in grant writing and reporting
Ability to lift 30-50 lbs
Availability to work evenings and weekends for program related events
EDUCATION AND/OR EXPERIENCE:
Bachelors degree in Social Work, Aging Services, Health Administration, or related field required
Community, case management, or outreach experience preferred
At least 3 years of prior job-related experience in a health care setting
At least 1 year’s experience as a manager/supervisor
