Supervising Medical Case Manager (LMSW)
Callen-Lorde Community Health Center of New York City – New York, NY

Callen-Lorde Community Health Center provides sensitive, quality health care and related services targeted to New York’s lesbian, gay, bisexual, and transgender communities regardless of ability to pay.

We are seeking currently seeking a Supervisor (SCM) to lead our Case Management team in the Supportive Care Coordination department. The SCM will perform grant management and supervise five case managers who provide medical case management in one of three areas: HIV Services, Transgender Services, or Women’s Health Services. The SCM will also have their own small caseload of patients. The Supportive Care Coordination department assists patients with accessing treatment education, supportive counseling, insurance, housing and entitlement programs. They also work closely with medical, mental health and outreach workers to ensure that the full continuum of care needs of our clients are identified and appropriate services accessed, coordinated, and monitored.

Work Schedule

Regular business hours, generally Monday-Friday 9:00-5:00pm. One evening shift (12:00–8:00 pm) will be required.

Qualifications:

  • Master’s degree in Social Work or Mental Health Counseling and NYS licensure – LMSW, LCSW, or LMHC
  • 2+ years of supervisory experience & high level administrative experience
  • Experience in HIV, medical/social services and entitlements work required
  • Prior grant management experience
  • Strong computer competency, including knowledge of Electronic Medical Records, Computerized Appointment/Billing Programs and other Microsoft Office Programs
  • Experience working with LGBT populations
  • Strong customer service orientation
  • Familiarity with LGBT, HIV, Transgender, Women’s Health and health-related issues and community resources
  • New York State HIV counseling and testing certification a plus
  • Bilingual (English/Spanish) preferred
