Participate in targeted outreach activities at hot spots for young black MSM to encourage individuals in getting tested and engaged in care. Collaborate with care managers, insurance specialists, and outreach staff to provide linkage to care and decrease barriers to access for medical care and support services.

Responsibilities
- Conduct Intake Assessments and Follow Up Assessments at a 80% rate of program targets
- Participation in community events, such as health fairs, LGBT events, testing events, etc.
- Provides presentations for internal and external programs to promote the program services
- Conduct rapid testing in the field and on-site, provide pre and post-test counseling and partner notification services in accordance with State and Federal regulations.
- Work together with peer support staff to educate clients on issues that concern persons who test positive for HIV , including providing support groups and bringing in outside speakers for clients.
- Assist with the recruitment for 3MV participants and facilitate the intervention.
- Assist with the planning and implementation of ballroom related events in order to gain access to and engage clients in the target demographic.
- Assists with development and implementation of program events
- Participate in bi weekly supervisory sessions with the Project Coordinator
- Complete documentation regarding client assessments, consent forms, and demographic and contact information.
- Conduct ongoing outreach and retention efforts via one on one contact and through program events.
- Re-engage clients for the required follow up assessment
- Participate in bi weekly staff meetings and weekly peer chart review meetings
- Perform other duties as assigned
Qualifications
- Good verbal and written communications skills
- Knowledge of HIV /AIDS, homelessness, substance use and mental illness, LGBT youth issues and YMSM networks/hot spots
- Good problem-solving skills and ability to work autonomously
- Good computer skills (Word, Excel, etc)
- Associates Degree preferred, High School Education required
