The primary function of the Outreach Case Manager will be to provide case assistance, information and referral, and benefit and entitlement services to SAGE Center participants outside of Manhattan. The Outreach Case Manager will also conduct on-going assessment of eligible participants and liaise with all SAGE Center sites. The Outreach Case Manager reports to the Director of Care Management Services and receives group clinical supervision from the Senior Director of Programs.

Services & Advocacy for GLBT Elders (SAGE) is the country’s largest and oldest organization dedicated to improving the lives of lesbian, gay, bisexual and transgender (LGBT) older adults. Founded in 1978 and headquartered in New York City, SAGE is a national organization that offers supportive services and consumer resources for LGBT older adults and their caregivers, advocates for public policy changes that address the needs of LGBT older people, and provides cultural competency training for aging providers and LGBT organizations, largely through its National Resource Center on LGBT Aging and its growing social entrepreneurship project. With offices in New York City, Washington, DC and Chicago, SAGE coordinates a growing network of 27 SAGE affiliates in 20 states and the District of Columbia.
SAGE was recently awarded funding by the New York City Council to expand its Innovative Senior Center model for LGBT older adults to four additional sites: Bronx, Brooklyn, Staten Island and Harlem.SAGE has established a new position—Outreach Case Manager—to assist constituents attending these new program sites.
Key Responsibilities:- Provide case management, and information and referrals to SAGE Center participants;
- Make home visits and provide case management to SAGE constituents throughout the city;
- Conduct intake and assessments for new clients, and reassessments for on-going clients;
- Conduct of benefit and entitlement eligibility assessments, and assist clients with public benefits applications and other entitlement programs and opportunities;
- Conduct group workshops/programs on a variety of benefit and entitlement programs and other applicable topics;
- Refer eligible participants to other SAGE services as needed/relevant;
- Lead and run support groups as needs are identified;
- Conduct advocacy for clients on an as-needed basis with other service entities;
- Collect all participant and service unit data as outlined by organizational policies;
- Enter all participant and service unit data in database as outlined by organizational policies;
- Perform tabling and other outreach at neighborhood events; and
- Perform other tasks and duties reasonably related to their job responsibilities
Required Skills/Qualifications
- B.A. in social work or a related field plus 2+ years of work experience. Substantial work experience (4+) in comparable case management work experience, and working knowledge of aging and /or LGBT community, may be substituted for degree;
- Passion for working with people from diverse backgrounds, and cultural and linguistic competency in regards to age, ability, sexuality, gender identity and expression, race, ethnicity, class and other characteristics that affect the lives of diverse older adults;
- Strong organizational skills with an ability to manage multiple cases and prioritize tasks;
- Strong written and verbal communication skills, including public speaking;
- Flexible and dependable – this position requires some flexibility around scheduling; some evening or occasional weekend hours may be required;
- Ability to work independently as well as a member of a team;
- Computer-literate, with knowledge of Microsoft Office (Outlook, Word & Excel);
- General working knowledge of NYC, its neighborhoods & communities; and
- Willingness and ability to travel throughout NYC via public transportation on a daily basis.
Strongly Desired Qualifications
- Bilingual (English/Spanish) preferred;
- Familiarity with social and aging services networks and supports; and
- Familiarity with LGBT aging issues preferred.
