Callen-Lorde is seeking a dedicated, flexible individual to join our staff as a Health Homes Outreach Navigator. The outreach navigator is responsible for providing outreach and engagement services, case management, patient advocacy, semi-annual psychosocial assessments, intake assessments and crisis intervention services to individuals accessing medical or mental health services and are living with a chronic illness. The Health Homes Outreach Navigator is responsible for ensuring the HIV Special Needs contract requirements are met; as well as, perform outreach and intake services to any patients assigned to Callen-Lorde’s Health Home from Mount Sinai. The Outreach Navigator will work closely with medical, mental health, and HIV outreach workers to ensure that the full continuum of care needs of our patients are identified and appropriate services accessed, coordinated, and monitored. The Outreach Navigator will offer administrative support to the Director of Care Coordination and the Manager of Intensive Care Coordination by performing data collection and review, filing organization, facilitating legal services, purchasing office supplies, and additional special projects.

Work Schedule/Salary/Benefits Callen-Lorde is open Monday through Saturday. This is a Monday-Friday work schedule and some evening hours are required. Salary will be accompanied by an excellent benefits package, including individual medical, dental and vision insurance, life insurance, short- and long-term disability insurance, generous paid time off, and optional 403B plan.

Qualifications
  • College degree (BA/BS) in health/social services strongly preferred; High School Diploma with 3 + years of case management experience considered.
  • Experience in HIV medical/social services, case management and/or entitlements work preferred.
  • Experience working with LGBT populations.
  • Strong customer service orientation.
  • Excellent interpersonal, patient relationship building, and sales skills.
  • Familiarity with LGBT- and HIV-related health issues and community resources.
  • New York State HIV Counseling and Testing certification a plus.
  • Bilingual (English/Spanish) strongly preferred.
  • Familiarity with developing excel database spreadsheets.
  • Commitment to the mission of Callen-Lorde Community Health Center.
  • Ability to work effectively with diverse populations, including lesbian, gay, bisexual, and transgender communities.
  • Ability to work within team-based structure.
  • Strong communication and interpersonal skills.
  • Computer literacy preferred and internet savvy.
  • Ability to handle multiple simultaneous tasks and remain calm, focused, and helpful under stress.
  • Excellent attention to detail.
  • Resourceful and persuasive.
