Under the direction of the HIV Services Manager, the MAI Team Leader is responsible for overseeing the day-to-day activities of MAI core services which include case management, care coordination, substance abuse counseling, client advocacy, and mental health treatment. The MAI Team Leader provides program administration, supervision of the multi-disciplinary team, and direct services to clients who are HIV+. The MAI Team Leader works closely with all regional Early Intervention Service Centers (EISC), Part C (medical) providers, and Case Management programs to ensure MAI clients are offered and if appropriate, receive auxiliary HIV services. Participates in the development and implementation of program policies and procedures; prepares operational analyses and reports, and audits current procedures to monitor and improve efficiency of operations. Cultivates and maintains communication and networking with multi-ethnic organizations in the HIV/AIDS community. Serves as a role model for cultural and linguistic competency for other staff to emulate. Maintain confidentiality at all levels.
Supervisor Case Manager
Essential Functions of the JobProgram Supervision (20%)
- Responsible for general oversight and administration of MAI Integrated Services, providing overall supervision, management and/coordination of MAI staff.
- Provide lead direction, monitors and evaluates work performance of multidisciplinary team, train staff, providing both positive and corrective feedback related to individual performance of assigned duties.
- Know the terms, conditions and scopes of work of contracts and grants that fund the MAI Integrated Services. Maintain and update policies and procedures for MAI Integrated Services.
- Provides quarterly chart reviews of all program areas (Case Management, Substance Abuse and Client Advocacy). Review all MAI Integrated Services assessments and Individual case plans to ensure accurate documentation and referrals.
- Ensure the timely implementation and coordination of all MAI Integrated Services. Timely and accurate completion of service statistics. Responsible for the preparation of monthly narrative and statistical reports.
- Oversee all client related educational activities including treatment updates, client orientation presentations, groups, etc.
- Attendance and participation in Departmental staff and supervisors’ meetings and required trainings.
- Coordinate quality improvement and evaluation activities for MAI related services for contract compliance and ensuring excellence in service delivery, and adequate progress toward contract goals and objectives.
- Participates in and coordinates the design and implementation of data collection and analysis systems for the unit, to include computer, and/or network systems, applications programs, and administrative procedures.
- Represent MAI Integrated Services and San Ysidro Health Center to other organizations and community groups. Network with other SYHC programs and HIV/AIDS service providers.
- Maintain a flexible schedule to allow for some weekend work as well as emergency coverage of shifts to meet the needs/demands of clients, related activities and events, and supervisory team.
Case Management (60%)
- Develop excellence in case management services which will enrich and enhance the quality of life, dignity and respect of people infected and/or affected by HIV/AIDS.
- Conduct a comprehensive assessment and administer an acuity scale to determine the medical, psychosocial, and social service needs of HIV+ clients.
- Ensure clients are enrolled in medical care and receive appropriate referrals to needed services as established in case plan. Services include (but are not limited to): medication assistance, housing, mental health counseling, substance abuse counseling, legal services, etc.
- Become an AIDS Drug Assistance Program (ADAP) enrollment worker and ensure all eligible clients complete the ADAP enrollment process.
- Maintains a caseload as assigned by contractual obligations. Conduct re-assessments and monthly contacts as required by contract. Assist clients in the procurement of benefits and services.
- Timely professional maintenance of records and documentation, including progress notes, treatment plans and ongoing evaluation of client progress, consistent with agency expectations and ethical standards.
Care Coordination (20%)
- MAI Supervisor performs thorough psychosocial assessments of incoming clients. Collaborates with clients to develop a personalized service plan to achieve self-reliance. Collaborates with MAI and other service team members and other appropriate individuals on clients’ status; within scope of responsibility and expertise, provides guidance and instruction to other staff members regarding appropriate treatments for clients.
- Facilitates coordination intra and inter-agency to assist clients in their provision of basic needs.
- Assists MAI Integrated Services in the promotion and implementation of client activities, educational presentations, and special events.
- Support and participate in activities and group events organized by MAI Integrated Services.
- Serve as primary liaison between the regional EISCs, EIPs, Part C, CM/PA, Mental Health and Substance Abuse programs ensuring the coordination of medical and social support services for MAI Integrated Services patients/clients.
- Participates in weekly HIV Multi-disciplinary/Case Conference meetings with Case Management/Care Coordinators, Mental Health Counselor, Substance Abuse Counselor, Client Advocate, and Part C clinical staff.
Job Requirements
Education Required (Minimum level of education): Bachelor’s degree required, Master’s Degree in social/human field preferred.
Certifications/Licenses Required: Current valid California driver’s license and access to insured vehicle required.
Experience Required (Minimum level of experience): three (3) years of experience working in human or social services. Experience with and sensitivity to LGBT-related cultural concerns and the psychosocial issues concomitant to HIV disease. Supervisory experience in a non-profit health or human service agency is highly desirable. Previous HIV field experiences a plus.
Verbal and Written Skills Required to Perform the Job: Bilingual English/Spanish required, with excellent oral and written communication skills. Communicate clearly and concisely, both orally and in writing.
Technical Knowledge and Skills Required to Perform the Job: Demonstrate ability to be culturally sensitive and appropriate in working with minority communities. Demonstrate respect for diversity. Knowledge of HIV/AIDS and its physical, psychological & emotional effects on individuals and family units. Knowledge of health and social issues people with HIV/AIDS. Excellent interpersonal skills. Exercise tact, objectivity, sensitivity, strategy and judgment in dealing with a variety of people with a variety of co-occurring disorders. Organize, set priorities and exercise sound independent judgment within areas of responsibility. Establish and maintain effective working relationships with professional staff, clients, family members, and the public.
