Supportive HIV Care Coordinator
Callen-Lorde Community Health Center is seeking a dedicated, flexible individual to join our staff as a Supportive HIV Care Coordinator (SCC). The SCC is responsible for providing case management, treatment education and supportive counseling services to HIV-positive individuals accessing medical and mental health services. The SCC will work closely with medical, mental health and outreach workers to ensure that the full continuum of care needs of our clients are identified and appropriate services accessed, coordinated, and monitored.
Work Schedule/Salary/Benefits Callen-Lorde is open Monday through Saturday. Some weekly evening hours (12 noon – 8 pm) will be required. Salary will be accompanied by an excellent benefits package, including individual medical, dental and vision insurance, life insurance, short- and long-term disability insurance, generous vacation/leave time, and optional 401K/403B plans.
Qualifications- Bachelors Degree in Human Services field. Masters preferred.
- Experience in medical/social services and entitlements work required.
- Strong computer competency, including knowledge of Electronic Medical Records, Computerized Appointment/Billing Programs and other Microsoft Office Programs.
- Experience working with LGBT populations.
- Strong customer service orientation.
- Familiarity with LGBT, HIV and health-related issues and community resources.
- New York State HIV Counseling and Testing certification a plus.
- Bilingual (English/Spanish) preferred.
- Must be internet research savvy.

Callen-Lorde’s grassroots heritage dates back over 40 years to the St. Mark’s Community Clinic and the Gay Men’s Health Project, two volunteer-based clinics that provided screening and treatment for sexually-transmitted diseases. These clinics merged in 1983 to form Community Health Project, a mostly volunteer-staffed, episodic care program housing the nation’s first community-based HIV clinic.
