Callen-Lorde is seeking a dedicated, flexible individual to join our staff as a Youth (T/GNC) Medical Case Manager within our Health Outreach to Teens, or HOTT program. The HOTT Program provides low- and no-cost medical and mental health care to LGBTQ and homeless youth, ages 13-24. The Medical Case Manager (MCM) provides a range of patient-centered services that coordinates all medical related services, diminishes barriers to care and facilitates the receipt of social and supportive services to achieve optimal health. The coordination and follow-up of medical appointments are key activities of MCM. These services ensure timely and coordinated access to health and support services, through ongoing assessment of patient needs and personal support systems. This includes the provision of treatment adherence counseling to ensure adherence to medications, as well as self-management skill development and health education. In addition, the T/GNC MCM will ensures medical case management services are available to the patient and are documented in the patient’s record, develop an initial psychosocial assessment of service needs of patient is conducted, identifies the needs, resources and strengths of the patient that form the patient’s individualized service plan.

Work Schedule/Salary/Benefits The Health Center is open Monday through Saturday day and evening. We are seeking someone who can commit to working approximately a 15 hours per week schedule as a per diem.
Qualifications- MSW preferred, college degree (BA/BS) in health/social services or the equivalent.
- Experience in medical/social services (case management) and or entitlements work preferred.
- Familiarity with LGBT, HIV and health-related issues and community resources.
- Strong computer competency, including knowledge of Electronic Medical Records, Computerized Appointment/Billing Programs and other Microsoft Office Programs.
- Experience in working with youth populations.
- Strong customer service orientation.
- Bilingual (English/Spanish) strongly preferred.
- Ability to work with diverse populations, ability to handle multiple, simultaneous tasks.
- Ability to work independently yet be team focused.
- Ability to remain calm, focused, and helpful under stress.
- Excellent attention to detail.

Health Outreach to Teens (HOTT)
Health Outreach To Teens is a welcoming, non-judgmental, confidential program designed specifically to meet the medical and mental health needs of lesbian, gay, bisexual, transgender, and questioning adolescents and young adults ages 13-24, as well as homeless youth, runaways, sex workers, squatters, and other street-oriented youth. These services are offered both at a youth-only medical suite at Callen-Lorde, and on HOTT’s medical van, which travels to areas throughout Manhattan in the afternoons and evenings where youth are known to hang out.
All of HOTT’s services are provided free of charge or at low cost (insurance is accepted). HOTT will register young people 21 years old and younger for health care services, and provide ongoing care up to their 24th birthday. After their 24th birthday, they will be transitioned into adult care.
